Medication Authorization Policy

Important licensing requirements for all Day School families

Day School Families,

Per our licensing, medications may not be administered without a doctor’s prescription AND a Parent/Guardian Authorization for Administering Medication Form (DHR-CDC-1949).

This includes all prescription and over-the-counter medications including, but not limited to: Tylenol, Motrin, all diaper creams, lotions, Vaseline, Aquaphor, sunscreens, lip balm, etc.

We encourage parents of children who frequently require medications (for example, diaper creams) to contact their child’s physician and request a School Medication Prescription. The doctor’s medication form must specify the exact medication your child uses. No blanket forms (such as “diaper ointment”) will be accepted.

This form will be valid for 12 months. Once we have this form on file, you may submit your child’s medication or diaper cream with a Parent/Guardian Authorization for Administering Medication Form (DHR-CDC-1949) as needed or every 7 days for ongoing use.

All medications must be in original packaging. Prescription medications must include the prescription sticker on the container (including EpiPens, Albuterol inhalers, etc.). The Parent/Guardian Authorization for Administering Medication Form (DHR-CDC-1949) is valid for 7 days from the date it is signed.

Please do not leave any medications in your child’s backpack. We recommend designating your child’s backpack for school use only rather than using it as a diaper bag. If medications arrive at school in a backpack, they must be picked up immediately or disposed of.
